If you are in need of biblical guidance to help you through the struggles and difficulties of life and desire to meet with one of our pastors or lay counselors, please download the Personal Data Form and the appropriate Consent to Counsel Agreement below. Once you have downloaded the Personal Data Form, please fill out the information in its entirety to the best of your ability. Then, please download and read the Consent to Counsel Agreement in its entirety and initial at the bottom of each page stating you have read and understand the contents of the agreement and then sign the document at the bottom. If you are requesting support on behalf of a minor, please also download the Consent to Counsel a Minor Agreement. After you have filled out the Personal Data Form and signed the Consent to Counsel Agreement, you may return the forms to our church secretary, Carla Massey, by mail or in person at 1290 Springwood Church Road, Gibsonville, NC 27249. After you have submitted these necessary documents, our church secretary will then schedule you an appointment to meet with one of our biblical counselors.
